NEW POSITION! JOB TITLE: Seniors Community Connector 


JOB SUMMARY:

The Seniors Community Connector (CC) will work with older adults who are referred from Health Care professionals and the Better At Home program for Daajing Giids and K’il Kun (Sandspit) to connect with community supports and services through applications, advocacy, and introductions. The role includes a diverse range of responsibilities- from conducting assessments and co-developing personalized care plans, to providing practical support to older adults and their families, to establishing, and maintaining relationships with people and organizations in the community offering supports to older adults. 


KEY DELIVERABLES:

To identify the support needs and interests of the seniors in the two communities by lessening senior isolation through group activities or to simply having a visit for a cup of tea/coffee. You will plan and execute learning opportunities, for example, Wills, preparing health care plans with a health care professional, creating a plan to manage or mitigate debt, budget their income, making sure they are claiming all of the benefits to which they are entitled. 

Your experience, training (provided) and interpersonal skills help you to make meaningful connections as you discern their unique needs. Through your work with local agencies and professionals, you will make a positive impact on the lives of older adults and the community. 


QUALIFICATIONS:

a combination of relevant education and experience which could entail a Batchelor’s degree in social service or related Human Services field and/or 

Strong knowledge of seniors’ issues and challenges related to healthy aging and/or 

Experience working with seniors and diverse populations from different cultures and socio-economic backgrounds and/or 

Knowledge of community resources, programs and services and/or 


Minimum two (2) years recent related experience ie caregiving 

 

KEY DUTIES:

Connect, liaise, and establish partnerships with local health care professionals to create and maintain referral pathways 

Understand hospital discharge procedures to assist supporting seniors transitioning back to their homes 

Build and maintain networks within community and strengthen relationships within the community-based senior serving sector including event planning (ie socializing opportunity/month) 

Use an asset-based community development approach to identify and mobilize individuals and organizations providing support to older adults 

Engage and participate in learning opportunities such as in community of practices, planning tables or networking events 

Report writing, computer and budgeting skills

This is a contract position for 25 hours a week. Wage is $30-34 per hour. Open until filled.
