Niislaa Naay Healing House Society is well funded and in the early stage of operating as an independent Society. Niislaa Naay services the community of Old Massett, as well the neighbouring communities. Niislaa Naay hosts many interactive community programs such as traditional wellness, mental health, community health, midwifery, maternal health, home care, youth services, dentistry, patient travel, and primary care services. Services are provided from three locations: the youth centre, wellness house, and the Niislaa Nay Healing House. Many programs that are offered have a land-based healing approach incorporating the culture and traditions of the Haida people.

Job Description

Salary: $32.00-$35.00 per hour

Location: Remote or 347 Eagle Ave, Old Massett, BC

Type: Part Time Permanent (15 hrs. per week)

Description

The Haida Gwaii Primary Care Network (PCN) is a partnership between Pacific Northwest Division of Family Practice, Northern Health Authority, First Nations Health Authority and local First Nation communities. The vision of PCN is to transform primary care for patients and providers, to ensure every interested patient can access longitudinal integrated team-based care in their Patient Medical Home. Our core values are access for all, culturally safe and appropriate care, and locally rooted care.


Reporting to the Health Director or delegate at Niislaa Naay Healing House Society, the PCN Coordinator supports the collaborative partnership and PCN goals established by the Haida Gwaii PCN Steering Committee in providing culturally safe, team-based care by Health professionals within Haida Gwaii communities. This position will be responsible for coordinating and implementing assigned related activities. This position also plays a critical role in successfully operationalizing the Haida Gwaii PCN. The role works with physicians, Northern Health Authority, First Nations Health Authority, and First Nation communities, and other stakeholders to increase access to quality primary care in Haida Gwaii.


Key Duties & Responsibilities

• Provides support to the PCN steering committee and other committees as assigned.

• Development and implementation of project work plans, coordinates project activities, and identifies strengths and gaps related to the implementation of the PCN.

• Supports member and stakeholder engagement activities

• Organizes and facilitates meetings; works with the partners on agenda setting, prepares and distributes meeting material, identifies, tracks, and follows up on action items.

• Provides reports on PCN implementation activities and outcomes and communicates progress of the PCN as directed by the Haida Gwaii Co-Chairs.

• Develops resources for members and the public, including manuals, communication briefs, and emails.

• Collaborate with colleagues to streamline and optimize operational processes and procedures.


Knowledge, Skills, and Abilities

• An understanding of the BC health system and primary care. Experience working directly in healthcare or supporting healthcare initiatives is an asset.

• Committed to cultural safety and to continuous cultural learning and to lifelong learning, with a passion to enhance patient care and provider satisfaction.

• Skilled at preparing for effective meetings. Proficiency in Word, Excel, Outlook and PowerPoint.

• Demonstrated understanding and application of quality improvement methodologies. Experience and training in change management principles is an asset.

• Ability to work effectively with deadlines, multiple projects and balance work priorities.

• Skilled at navigating a variety of personalities and social circumstances.

• Travel within the region will be required.
